About the role

Kestrel's engineering team builds software that medical practices rely on every day to get patients the care they need. Our stack is Next.js 15 (App Router), Supabase (Postgres + Auth + Edge Functions), Tailwind CSS, and Framer Motion on the frontend, with AI-powered clinical extraction pipelines and FHIR R4 integrations on the backend.

As a full stack engineer, you'll work across the entire stack: building new features, improving performance, integrating with EHR systems, and shipping reliable software that handles sensitive healthcare data. You'll work directly with the founder and have significant influence over technical direction.

Minimum qualifications

  • 2+ years of professional software development experience (or 1+ years with an advanced degree).
  • Proficiency in TypeScript and React. Experience with Next.js (App Router) is strongly preferred.
  • Experience building and consuming RESTful APIs.
  • Familiarity with relational databases (Postgres preferred), including writing queries, designing schemas, and understanding indexing.
  • Comfort working in a fast-moving environment where you own features end-to-end, from design to deployment.

Preferred qualifications

  • Experience with Supabase, Firebase, or similar BaaS platforms.
  • Familiarity with healthcare data standards (FHIR R4, HL7v2, X12 EDI).
  • Experience integrating with third-party APIs, especially in healthcare (EHR systems, clearinghouses, payer portals).
  • Experience with AI/ML APIs (OpenAI, Anthropic) for structured data extraction or NLP tasks.
  • Familiarity with Tailwind CSS, Framer Motion, or similar frontend tooling.
  • Understanding of HIPAA compliance requirements for software handling PHI.
  • Experience with CI/CD pipelines, Vercel deployments, and automated testing.
  • You've worked at an early-stage startup and thrived in the ambiguity.

Responsibilities

  • Design, build, and ship features across the full stack: frontend UI, API routes, database migrations, and background jobs.
  • Build and maintain integrations with EHR systems using FHIR R4 and other healthcare data standards.
  • Develop and improve AI-powered clinical extraction pipelines that read clinical notes and populate prior authorization forms.
  • Write clean, well-tested, secure code that handles protected health information (PHI) responsibly.
  • Participate in code reviews and contribute to engineering best practices.
  • Debug production issues, optimize performance, and improve system reliability.
  • Collaborate with design and clinical operations to translate user needs into technical solutions.
  • Contribute to technical architecture decisions as the platform scales.

Compensation

Base salary: $120,000–$180,000 + equity + benefits. Compensation depends on experience and location.
